Medical Chronologies for Personal Injury Cases: What to Include and How to Outsource Them

By Avish Sharma, advocate. Published November 8, 2026. 5 min read.

In a personal injury, no-fault, workers' compensation or medical malpractice case, the medical records are the case. They also arrive as hundreds or thousands of pages from different providers, out of order and full of repetition. A medical chronology turns them into a single dated account an attorney can rely on, with every entry traceable to the page it came from.

What a Medical Chronology Is

A medical chronology is a table of every relevant medical encounter in date order: who saw the client, what they found, what they diagnosed, what they did, and what they planned next. Unlike a narrative summary, it is built to be checked: each row cites the record and page, so the attorney, an expert or opposing counsel can go straight to the source.

The Columns a Good Chronology Includes

  • Date of service.
  • Provider and facility.
  • Type of encounter: emergency visit, office visit, therapy, imaging, surgery, independent medical examination.
  • Subjective complaints, as the patient reported them.
  • Objective findings and test results.
  • Diagnoses, with ICD-10 codes where they appear in the record.
  • Treatment, procedures and medications.
  • Work status, restrictions and the treatment plan, including referrals and future care.
  • Source: the record name and page or Bates number.

What the Chronology Should Flag

The value of a chronology is in what it surfaces for the attorney, not only in what it lists:

  • Gaps in treatment, which the defence will use to argue the injury resolved.
  • Prior injuries and pre-existing conditions affecting the same body part.
  • Inconsistent histories, where the patient described the incident or symptoms differently to different providers.
  • Statements about causation, and any provider's opinion on permanency or maximum medical improvement.
  • Recommendations for future treatment, which drive the damages model.
  • Missing records: a referral with no matching visit, or an imaging study with no report.

How Attorneys Use It

The same chronology works across the life of a case. It supports the demand letter and the damages calculation, prepares the attorney to depose treating physicians and to cross-examine the defence's examining doctor, gives retained experts a fast way into the records, and becomes the backbone of mediation statements and trial exhibits. A billing summary of the medical specials alongside the chronology completes the picture.

Protecting the Records

Plaintiff firms usually obtain records with the client's written HIPAA authorization, and a law firm is generally not itself a HIPAA covered entity. Different rules apply when a firm handles records for a covered entity, for example defending a hospital or provider: HIPAA's definition of a business associate expressly includes legal services, and anyone the firm passes those records to may need to sign a subcontractor business associate agreement.

Whatever the setting, medical records are among the most sensitive files a firm holds. Before outsourcing chronologies, require an NDA, a business associate agreement where the matter calls for one, access through your own systems rather than email attachments, and clear rules on where files may be stored and when they are deleted.

Using AI on Medical Records

AI tools are good at pulling dates, providers and diagnoses out of typed records, and they make a first draft of a chronology much faster. They also misread handwritten notes and scanned pages, merge separate visits and occasionally state findings the record does not contain. Every row of an AI-assisted chronology should be checked against the cited page by a person before the attorney relies on it.

How Prodigy LPO Prepares Chronologies

We prepare medical chronologies for personal injury, no-fault, workers' compensation and medical malpractice firms. Every entry cites its source page, gaps, prior conditions and missing records are flagged for the attorney, and a lawyer checks the chronology against the records before delivery. We sign your NDA, and a business associate agreement where you need one, before we see any record. Your first chronology is a free sample.
